HealthPathways is both a model of working; bringing together clinicians across the health care sectors to co-develop pathways, and a tangible product in the form of an online health information portal. 

The HealthPathways SA team consist of GP Clinical Editors and Clinical Coordinators who – 

  • Develop pathways of care
  • Conduct consultations prior to launch 
  • Review pathways to maintain accuracy and quality 

SA Health, Adelaide PHN and Country SA PHN are working in partnership to implement HealthPathways across the state to support consistent care and management of health conditions and to improve the health outcomes and journey of patients through our local health system. 

Who can use HealthPathways?  

HealthPathways SA is designed to be used at the point of care by GPs, however is also available to health care professionals across the state for use within their scope of practice.  

HealthPathways SA is not designed to be used by the general community. Depending on the specific pathway, relevant patient resources are available on the portal for health professionals to offer to patients.  

Access to HealthPathways 

If you are a health professional in South Australia and would like to access the HealthPathways SA website, please register for access via the Portal.

Consultation

Prior to any pathway being published to the live site, we undergo a consultation process.  

Consultation is a critical part of the HealthPathways where health professionals and service providers from across the SA are invited to consult on draft pathways. Some clinicians prefer to provide more global feedback, whereas others provide very detailed suggestions on specific clinical points or service information, depending on their professional role and familiarity with the topic / services.
