Integrated Care
Adelaide PHN is investing in innovative and integrated care models in order to better meet the needs of people living with chronic and complex conditions locally and to support the building of a more effective local health system. Elements of integrated care underpin all Adelaide PHN programs and commissioned services and are expressed through Adelaide PHN’s Integrated Care Framework.
General Practice Integration Units
General practice Integration Units (GPIU) are a partnership with the Local Health Networks (LHN) to improve health outcomes for patients between primary and tertiary care.
GPIUs (formally GP Liaison Units – GPLU) were established in 2018 to enhance partnerships between the LHNs and general practices. Since their inception, GPIUs have worked towards identifying and developing sustainable clinical collaborations, professional partnerships, service improvements and administrative efficiencies, with the aim to improve patient care between general practice and hospitals in our catchment.
The programs aim to reduce the number of avoidable hospital presentations and inappropriate referrals across the Royal Adelaide Hospital, Queen Elizabeth Hospital, Flinders Medical Centre, Noarlunga Hospital, Lyell McEwin Hospital, and the Modbury Hospital. The program also supports improved communication, collaboration and integration between general practice and the hospitals to improve health outcomes and experience for patients identified in the project cohort.
This unit is a joint project with the Northern Adelaide Local Health Network (NALHN) and aims to reduce the number of avoidable hospital presentations and inappropriate referrals to the outpatient department at the Lyell McEwin and Modbury Hospitals.
The unit can provide a direct clinical contact point for advice and assistance to navigate the hospitals and links directly with their medical teams.
This dedicated service is for GPs and their staff and is not intended for patients.
GPs can contact the unit to assist with urgent request for medical information or in coordinating community-based patient care and promote hospital avoidance pathways.
