Australia’s ageing population is increasing frailty, linked to disability, functional decline, higher healthcare costs, and poorer quality of life. General practice is well placed to identify and manage frailty early. The Adelaide PHN evaluated the Early Intervention Initiatives Phase 2 project (Dec 2024–Nov 2025), which supported 16 practices to screen at-risk patients (≥65 years; ≥55 for Aboriginal and Torres Strait Islander people) and deliver care planning over a 3‑month period (Feb–May 2025).
