Health Equity Awards 2024

The Health Equity Awards 2024 will recognise outstanding organisations and individuals for their efforts to continuously improve equity and access for vulnerable communities in the Adelaide metropolitan region.

These awards will recognise organisations and/or individuals who have:

• Improved health outcomes in our region. 
• Achieved equity for vulnerable populations groups through improved access and outcomes. 
• Improved patient journeys of care through alliances and partnerships with key primary care stakeholders in the local region.

 

Our Way of Thinking

The development of criteria, assessment matrix, and branding for this year's Awards was driven primarily by our focus on health equity.

By using a variety of graphic heights to symbolise health equity, our logo aims to demonstrate equity, as illustrated below, where various individuals require various supports to achieve their optimal state of health.

Awards are open to all current Adelaide PHN commissioned service providers, Adelaide PHN partners and general practices or those that are part of our Advisory Councils’ activities.

The achievements described in the nomination must have been delivered within the Adelaide PHN region, be current and have taken place, or been developed in, the last financial year.

We would like to hear about your and your team's impact on improving community outcomes and experiences. 

You are required to submit a case study as part of your application. The requirements are outlined in the Application Guidelines. Please refer this document while preparing your case study.

• Completed entries via the nomination form must be received by 9 pm on Friday, 24 May 2024.
• Entries received after this time will not be considered for the awards and no responsibility will be accepted by Adelaide PHN for lost, late, incomplete, or incorrectly submitted entries. 
• Only completed entries will be accepted.
• Case Studies are permitted to be up to 1500 words.
• Self-nominations are acceptable. 

The awards will be presented at the Adelaide Festival Centre on Saturday, 31 August 2024. The Award recipient(s) will be presented with a trophy. 

They will also be featured in a short video (showcasing their achievements) presented on the night, have their picture taken and an article featured in our publications.
