Adelaide PHN’s 2021 Annual Primary Health Care Award Winners Announced


Adelaide PHN is excited to announce the winners of our 2021 Annual Primary Health Care Awards. 

This year, Adelaide PHN looked to recognise the hard work and dedication of our region’s primary health care organisations, who have been responsive to our community’s needs in an ever-changing environment. Award categories and winners are listed below

Outstanding Achievement in Primary Mental Health Care Services

  • Winner: Neami National – Wellness Connect, Continuity of Support Program 
  • Runner up: Sonder – headspace Adelaide Primary Platform 

Outstanding Achievement in Primary Alcohol and Other Drug Treatment Services

  • Winner: Uniting Communities – Streetlink Youth Health Service 

Outstanding Achievement in Aboriginal Health

  • Winner: The ACT Centre – Aboriginal Community Connect, Mental Health Intervention 
  • Runner up: Sonder – Closing the Gap/Integrated Team Care Program 

Outstanding Achievement in Aged Care

  • Winner: ACH – COVID Resilience and Reconditioning Packages for Older People Impacted by the COVID-19 pandemic 
  • Runner up: BMP Healthcare Consulting – FORTRESS Frailty Education Program 

Outstanding Achievement in Digital Health and General Practice

  • Winner: Mawson Lakes Healthcare
  • Runner up: Arkaba Medical 

Outstanding Achievement in Workforce

  • Winner: Drug and Therapeutics Information Service (DATIS) – Quality Use of Medicines in Older Persons 
  • Runner up: Pharmaceutical Society of Australia (PSA) – COVID Vaccination Training and Development in Community Pharmacists 

Outstanding Achievement in Population Health / COVID-19

  • Winner: North Eastern Respiratory Clinic / Northern Vaccination Centre 
  • Runner up: Survivours of Torture and Trauma Assistance and Rehabilitation Service (STTARS) and Australian Refugee Association (ARA) - Adelaide Refugee and New Arrivals Program (ARANAP)

The 2021 winners and runners up have each received professional development sponsorships up to the value $3000 and $1500 respectively, to continue to build team and service capacity.
