Commissioning Soon: Care Finder Program


Adelaide PHN will soon be inviting applicants from suitably qualified and experienced organisations to deliver the care finder program and will be holding a Tender Briefing to provide more information amd support potential providers in preparing their application. 
 
This program aims to deliver specialist and intensive assistance to help people within the target population to understand and access aged care and connect with other relevant supports in the Adelaide PHN region.

The program will specifically target senior Australians who have one or more reasons for requiring intensive support to interact with My Aged Care (either through the website, contact centre or face-to-face in Services Australia service centres), access aged care services and/or access other relevant supports in the community.

The services are to be delivered for two and a half years from 1 January 2023 to 30 June 2025.

The services will be commissioned via an open tender process (Request for Proposal (RFP)) which we anticipate releasing in the coming weeks. 
 
We encourage all interested applicants to register to receive alerts via Adelaide PHN's Tenderlink portal.

Potential applicants that download the documents from Tenderlink will be sent an invitation to register for the Tender Briefing. The Briefing will provide an overview of the RFP, objectives and outcomes sought, the process for submission and to allow potential applicants to ask questions.
