Palliative Care Connect


Adelaide PHN are proud members of the SA Palliative Care Navigation Pilot Steering Committee supporting SA Health in shaping and embedding Palliative Care Connect within the local health system to deliver better outcomes for people in need. 

Palliative Care Connect is a new phone service for doctors and health professionals, aged care professionals, as well as carers, family members and patients that provides information and linkage to palliative care and bereavement supports in South Australia, with funding from the Australian Government. 

Palliative Care Connect Navigators are registered nurses and social workers who assist callers to coordinate and link people to appropriate services and assist with smoother transitions between types of care and into end-of-life care pathways. 

To date, Navigators have played a crucial role in supporting health professionals. They help clinicians to better understand services available to their patients, navigate complex healthcare and aged care systems, and collaborate effectively on palliative care and end-of-life planning. Through their expertise, Navigators enhance patient outcomes and foster stronger collaboration among healthcare teams. 

In addition to the phone line, the Palliative Care Connect website, developed in partnership with Flinders University, features key information about palliative care and bereavement. The website is a resource for GPs, practice nurses and other primary health professionals to refer patients to, and includes a self-assessment tool to help people identify the support and resources they need. The website’s service directory is available to help anyone find local palliative care, bereavement, and related community services. 

Other Palliative Care Connect services include: 

  • A regional model of palliative care navigation for services for the Yorke Peninsula consumers, health and aged care professionals (both in-person and via telehealth) in Clare, Kadina, Port Pirie, and surrounding areas.  
  • Bereavement navigation services statewide via telephone and in-person in Adelaide, Barmera, Berri, Gerard, Loxton, Mount Gambier, Paringa, Port Augusta, Renmark, Waikerie, and Whyalla.  
  • Aboriginal-specific navigation service (in-person in Port Augusta and Whyalla and via telephone statewide) to assist Aboriginal communities and health professionals to access and provide culturally appropriate palliative and end of life care.
