Care Connections is one of Adelaide PHN’s flagship integrated care programs and is being delivered in partnership with local general practices working in targeted, high need areas within metro Adelaide. The program brings together a suite of designed and commissioned activities that promote person centred care and foster communities of practice across the medical neighbourhood with clearer navigation, proactive communication and better articulation of roles. The latest iteration of this program focuses on supporting community dwelling people aged 60 years+ to stay well and out of hospital.
The Care Connections program is not set up for referral. Instead, particpating general practices work within areas of need and with identified patient cohorts.
