Central Adelaide General Practice Integration Unit (CAGPIU)
This unit is a joint project with Central Adelaide LHN to improve health outcomes for patients moving between primary and tertiary care. This program aims to reduce the number of avoidable hospital presentations and inappropriate referrals to the outpatient department at the Royal Adelaide Hospital and Queen Elizabeth Hospital from general practice, support improved communication, collaboration and integration between general practice and the hospitals, and improve health outcomes and experience for patients identified in the project cohort.
