Northern Adelaide General Practice Liaison Unit (NAGPLU)

This unit is a joint project with Northern Adelaide LHN to improve health outcomes for patients moving between primary and tertiary care. This program aims to reduce the number of avoidable hospital presentations and inappropriate referrals to the outpatients department at Lyell McEwin Hospital from general practice, support improved communication, collaboration and integration between general practice and Lyell McEwin Hospital, and improve health outcomes and experience for patients identified in the project cohort.
